Word Version of End User Certification Agreement On September 30, 1997, The Consumer Credit Reporting Reform Act of 1996 (the "Reform Act") became effective. The Reform Act added to the Fair Credit Reporting Act ("FCRA") a new section 604 (b), governing the use of consumer reports for employment purposes. This act states that before pulling a credit report, the end user must provide a "clear and conspicuous" written disclosure to the consumer in a document that consists solely of the disclosure, that a consumer report may be obtained for employment purposes, and obtain a written authorization from the consumer to pull his or her credit report.
